HOW TO RETURN A PART TO SILICON DESIGNS INC

1. Please read our Warranty Terms regarding returns.

2. Call Silicon Designs at 425-391-8329 or e-mail the Support department at . Please provide model and serial #. We will issue an RMDR #. This will be your reference to track the return.

3. Fill out the

with as much detail as possible. If you are returning failed units, please provide a technical contact person within your organization, ideally the person who determined the part is defective.

4. If you are returning a module for calibration, please provide a mating connector if you have installed your own plug.

5. For warranty items please remove any plugs / sockets that you have installed on the wiring or circuit boards attached to LCC or JLCC devices.

6. Place the devices in ESD bags and ship them with the Return form as soon as possible to the address on the return form.

7. Mark the RMDR# on the outside of the box to ensure correct routing.

PLEASE NOTE: Products returned without an RMDR # reference run the risk of being lost. After we have tested the returned parts we will try to contact you. If there is no response, items left at our facility for more than 180 days will be considered abandoned.

Product Return Terms (non-warranty returns):

Unopened Products only (ESD seal intact).
Subject to 20% Restocking and Re-calibration charge.
Only standard commercial accelerometers and modules may be returned.
Returns may be made for up to six months after original shipment date.
Please provide Purchase Date, Model Number, and Unit Serial Number when contacting us.
